The Community Care Access Centre has begun the “Care Connector” program… helping to hook up families with a doctor… or primary health care provider.
If you are without a family physician… and would like some help finding one… call 1-800-445-1822 to register… and those who need health care services the most will be helped first.
Care Connectors… who work closely with the South East Community Care Access Centre… have already matched more than 3,500 individuals with a primary care provider.
