Campbellford Memorial Hospital (CMH) has entered into an expanded partnership with the Toronto Grace Health Centre that will help patients awaiting long-term care.
The Remote Care Monitoring Plus (RCM+) program provides eligible CMH patients with remote care monitoring and personal support worker (PSW) assistance to ensure a safe discharge home for patients waiting to get into long-term care facilities.
CMH applied for and received funding for the program in 2023.
The RCM+ program equips patients with a pendant that uses geolocating technologies to detect wandering or falls.
It also provides extensive PSW support and access to nursing care through Home and Community Care services.
“We are thrilled to offer the RCM+ program to our patients,” said Heather Campbell, VP of Patient Care at Campbellford Memorial Hospital. “This innovative program has proven to be a safe and effective care option, allowing eligible patients the ability to stay at home while waiting for long term care.”
